Tobacco Use Surcharge:

If you attest that you are a tobacco user, a surcharge of $25 per paycheck will be added to your medical premium contribution.

Who is considered  a Tobacco or Nicotine User:

Anyone who currently uses cigarettes, e -cigarettes, chew, cigars and any other tobacco-related materials. This includes regular tobacco usage and occasional tobacco use during social  occasions.

Removal of Tobacco Surcharge:

The tobacco surcharge may be removed by completing an approved tobacco cessation program. Once you have completed a tobacco cessation program and provided proof of completion, the surcharge will be removed by the first of the following month from your completion date.
