The Elite EPO has the lowest deductible and out-of-pocket maximum, but costs the most on a per-paycheck basis and your care is limited to the Anthem Blue Cross network. Services sought outside of the Anthem network will not be covered, except for life-threatening emergencies.

 

The EPO plan allows you to seek treatment with a specialist within the Anthem network without having to acquire pre-authorization .

On the EPO plan, deductible and out-of-pocket maximums are embedded, which means that claims paid towards an individual member’s $500 deductible will switch to coinsurance once the deductible is satisfied, even if on a plan with family members
