Change for 2019 we will be utilizing CVS RX Network for Prescriptions! loanDepot will utilize the CVS prescription network starting in January, 2019. What that means is that you need to fill prescriptions in the CVS network, which has over 74,000 pharmacies across the U.S., including CVS/pharmacy®, Target, Costco, Rite Aid, Walmart, etc. If you currently use a pharmacy outside the CVS network, you may transfer over any prescription by taking your Rx bottle to a CVS network pharmacy, and they can get it transferred over. Additionally, if you take any prescriptions on a regular, repetitive basis, we recommend that you utilize the services of the Optum Rx mail-order pharmacy, which provides the highest level of benefit. When you use the mail order pharmacy, you get a 90 day supply of your medication for the price of a 60 day supply!

  • The PPO plan gives you the freedom to seek care from any licensed provider
  • For in-network office visits and prescription drugs, you will just pay a copay
  • For most other services, such as diagnostic lab work and imaging, you will pay the full amount until you meet the deductible, then you will pay a 15% coinsurance and the plan will pay 85% until you reach the annual out-of-pocket maximum.
  • Deductibles and out-of-pocket maximums are embedded. This means that aside from the employee only coverage level, once an individual on the plan has met the $1,000 deductible, the plan will start to pay 85% of the cost for services, even if the full family deductible has not been met.
  • Keep in mind that you generally pay less when you receive care from doctors and hospitals that participate in the UHC Select Plus network. These providers have agreed to provide services to plan members at contracted rates. The deductible and out-of-pocket maximums are also higher for out-of-network services

Mail Order 90 Day Supply When you use the OptumRX mail order program to refill your prescription, you receive a 90 day supply of your medication for the cost of a 60 day supply!

*Specialty RX drugs are subject to a separate $3,500 out of pocket maximum. After you pay $3,500 in copayments for Specialty drugs in a calendar year, you will not need to pay any more copayments for these types of drugs for the rest of the year.
